The medical world has made astounding progress in the past decade. Tools available to the caregiver are improving constantly, increasing diagnosis and treatment options. This trend has significantly increased total patient medical data. Concurrently, patients are becoming increasingly mobile and are seeing more caregivers than in the past. Together, both trends of increased medical data and increased patient mobility have resulted in the creation of numerous partial medical records scattered among physicians, systems and institutions vs. one comprehensive medical record.
